I want Magen David Adom UK to treat as GIFT AID DONATIONS all donations I have made for the past four years, and all donations I make from the date of this declaration until further notice unless I notify you otherwise in writing. I confirm that I am a UK taxpayer and understand that if I pay less Income Tax and/or Capital Gains Tax in the current tax year than the amount of Gift Aid claimed on all my donations it is my responsibility to pay any difference.
